Eligibility Requirements
Are you or a family member eligible for Home Care Services?
- A qualifying physician will order the home health care you or your family member needs.
- The patient must be meet the Medicare homebound guidelines. The patient must need one of the following:
* Intermittent Skilled Nursing
* Speech Therapy
* Physical Therapy
* Continue to need Occupational Therapy
- The home Health Agency must be Medicare Certified
Medicare will cover your home health services for as long as you are eligible and your physicians says you need the service(s). We will work closely with your physical to ensure that you continue to receive all services that are needed.
